Share a form on your website
Attach an active form to a website button, check the visitor path, or share its link or QR code.
Add an active form to a website button, publish the page, and follow the same path a visitor will use.
Before you share
Forms are included with Website and Full. Confirm the form shows Active under Communication → Forms → Manage Forms. Also make sure a helper has both Forms access and a personal subscription by following Give a helper notifications and access to responses.
Adding the button requires a role allowed to edit website content. Publishing the change requires website publishing access. Those permissions are separate from the Forms Manager role.
Add the form to a Bento Grid home-page button
This button editor is available in home-page content sections when the site uses Bento Grid. To identify the current layout, see Choosing a Website Layout.

Use the contact button in another layout
Other home-page layouts can show a configured contact button instead of Bento Grid's content-section buttons. In the editor, look for Show Contact Button, choose the active form under Form, and edit Button Text. If your layout does not show those controls, share the direct link or QR code below. You do not need to switch layouts to make the form available.
Share a direct link or QR code
On Manage Forms, open the three-dot menu for the form. Use Copy Link for email, text or social media. Use QR Code, then Copy URL or Download, for printed material. Open the copied link once before distributing it. When the form has a Redirect URL, this direct public page follows it after a successful response; the website button dialog uses the confirmation message instead.
Optional: start a workflow
Full includes Workflows for a repeatable, multi-step care process. A person with workflow-management access can create or edit a workflow, choose When a form is submitted, select this form, add at least one step, and activate the workflow. Follow Workflows for setup and operating instructions. A workflow is optional; it is not required to receive or review form responses.
