Create a form that is ready for responses

Build a focused form, choose what happens after submission, and prepare a helper before sharing it.

Create a short form, make its successful-submission result clear, and leave it active so people can respond.

Before you start

Forms are included with Website and Full. Sign in to the correct church with the Forms Manager role, or another role with Manage forms access. A desktop or laptop gives the form builder more room.

Build the form

Open a new form

In the admin navigation, open Communication → Forms, then select New Form. Give the form a name that tells staff what the response is for, such as “Prayer request” or “Plan a visit.”

Use the right starting fields

A new custom form starts with a Name field. If you entered the builder from a contact, prayer request, first-time visitor, or waiver template, review its prefilled fields. Keep only what your helper needs to respond.

Add and edit fields

Use Quick Add for common fields, or choose a field under Field Types. Select a field card to change its label, placeholder and Required setting in Field Properties. Close that panel before adding another field. Choice fields also let you edit their options. Use the arrow controls to reorder fields and the copy or trash controls to duplicate or remove one.
Keep the field list focused and use Live Preview to check what a respondent will see.

Tip

Ask for an email address or phone number when a person expects a reply. The helper cannot follow up from an anonymous response with no contact method.

Set the result and save

Choose the form type

Open Settings and choose the form type that best describes its purpose. The type helps staff recognize the form in the Forms list; it does not add or replace fields.

Write the confirmation

Leave Redirect URL empty to show a Confirmation Message after a successful response. Say what happens next without promising a response time your team cannot meet. Use a redirect only when the respondent must continue to another page from the form's direct public link. A form opened from a website button stays in its dialog and shows the confirmation message.

Keep it active and save

Under Form Status, leave Active Form on, then select Save Form. You return to Your Forms, where the form should show Active.

Note

An inactive form stays in the Forms list but does not accept new responses. Use that state when you want to pause collection without deleting the form or its history.

Prepare someone to receive the first response

Before you publish a link, follow Give a helper notifications and access to responses. Access to Forms and a personal notification subscription are separate. Then add the form to your website or share its public link.
